Photo ID

Driver license, state ID, passport, military ID, Tribal ID, or another accepted identity document.

Benefit proof

Medicaid, SNAP, SSI, FPHA, Veterans Pension, survivor benefit, or qualifying Tribal program document.

Income proof

Tax return, benefits statement, unemployment statement, pay stubs, or other official income proof if applying by income.

Address proof

Lease, utility bill, government ID, tax return, or another accepted address document if requested.
